Cash flow: a scanner doesn't bill on day one

New equipment in a practice means install, calibration, staff training, workflow changes, and then — eventually — production. New practice or new associate coming on? The equipment purchase and the revenue that justifies it are often six months apart. Structure the deal for the gap, not for the ideal.

New vs. used in healthcare

Buy new when the technology is the product. Imaging, CAD/CAM, aesthetics lasers — these evolve fast, patients notice, and being a generation behind is a competitive problem, not just an equipment problem. Warranty and manufacturer support matter enormously when a failure means canceling a day of patients.

Buy used when it's furniture and fundamentals. Chairs, cabinetry, sterilizers, compressors, exam tables, vacuum systems. This equipment is built to last decades, and practice closures and consolidations put excellent units on the market regularly. A refurbished operatory package at a fraction of new does the same dentistry.

Used qualifies for the same first-year write-off as new — bonus depreciation requires first use by your business, not first use ever.

Leasing is worth a serious look here, more than in most industries. If the equipment goes obsolete before it's paid off — which describes a lot of imaging and aesthetics — leasing keeps you current, keeps the payment lower, and gives you an upgrade path at the end of term. Ask us about it.

Section 179: deduct the equipment this year

Qualifying equipment placed in service before December 31 can be written off in full in year one under Section 179 and 100% bonus depreciation — on financed equipment, not just cash purchases.

Example: you finance a $110,000 CBCT unit in September and have it installed and scanning in November. You may be able to deduct the full $110,000 against 2026 income while a fraction of that has actually left your account.

For practices structured as pass-throughs, the interaction between Section 179, bonus depreciation, the taxable income limitation, and your personal return is genuinely worth a real conversation with your CPA — not a quick email. There's meaningful money in getting the ordering right.

The equipment must be placed in service — installed and ready for use — by December 31. For anything requiring construction, plumbing, lead shielding, or a state inspection, work backward from that date generously.
